Appointment Process:

You will receive an immediate automated email response from Lifegate.
Our Administrator will contact you via email within 2-3 business days.
You will be offered an appointment date and time.
You are responsible to confirm the appointment within 48 hours via an email response to Lifegate.
A confirmation email will be sent to you with important documents for you to read prior to your appointment.
Location and directions to our office are included in the email and documents.
Due to our waiting list, it may be several weeks between your confirmed appointment and your initial request.
